This section sets out both the annual performance reports as well as the past performance framework reports.
The Falkirk Health and Social Care Partnership’s Annual Performance Report for 2016 – 17 shows the progress that the Partnership has made in delivering its Strategic Plan during the first year of integration. The report also sets out the work planned for the year ahead.
The report does not include all the work that is happening across the Partnership but instead highlights areas of change and innovation. During the course of the year, the Integration Joint Board will receive many reports and briefings covering the full range of work that the Partnership undertakes.
The Annual Performance Report highlights the complex and challenging environment of health and social care. The Falkirk Partnership has been able to respond to, and plan ahead for many of these challenges. This is due to the commitment, resilience and professionalism of our workforce and our Partners, including carers and volunteers. The Annual Performance Report shows how by working together, we are making a positive difference to people who use our services and their carers.
Falkirk Integration Joint Board receives regular Performance Reports. Links to these reports are provided below. These reports ensure the Board fulfils its ongoing responsibility to ensure effective monitoring and reporting on the delivery of services. They also provide information on performance against targets and measures set out in the Strategic Plan.
A Strategy Map has been created, which ensures there is a direct link from National Health and Well Being outcomes and Strategic Plan outcomes to performance indicators.
