The Falkirk Health and Social Care Partnership’s Annual Performance Report for 2016 – 17 shows the progress that the Partnership has made in delivering its Strategic Plan during the first year of integration. The report also sets out the work planned for the year ahead.
The report does not include all the work that is happening across the Partnership but instead highlights areas of change and innovation. During the course of the year, the Integration Joint Board will receive many reports and briefings covering the full range of work that the Partnership undertakes.
The Annual Performance Report highlights the complex and challenging environment of health and social care. The Falkirk Partnership has been able to respond to, and plan ahead for many of these challenges. This is due to the commitment, resilience and professionalism of our workforce and our Partners, including carers and volunteers. The Annual Performance Report shows how by working together, we are making a positive difference to people who use our services and their carers.
The purpose of this report is to present performance in relation to unscheduled care and delayed discharge, as part of a more targeted approach to performance reporting on thematic areas and in support of the Winter Plan. The reported partnership indicators are linked to the outcomes of the Strategic Plan.
